I purchased this book because I lost my 39-year old son 2 months ago from a heroin overdose. He had a 25-year history of drug addiction, has been to treatment 6 times, and when sober - which he was for years at a time - was active in his recovery of drug and alcohol abstinence, 12-step meetings, and working with a sponsor. I wanted to understand where we went wrong. After he died, I started searching the internet, listening to Ted Talks, watching YouTube videos, and reading everything I could find about the current science of addiction. As providence would have it, I found Dr. Grisel's book on Amazon. I just finished it this morning. I'm a health care provider myself (nurse practitioner) and a science geek; I really wanted to understand what happened. Dr. Grisel nailed it. My son was a risk taker from day one – he had no fear, and was always looking for new adventures. He was always a gifted athlete and good student – he was good at almost everything he tried, and he was enthusiastic about all things new. He was also an extrovert and loved the thrill of a good party. He started experimenting with alcohol and drugs when he was 14. Dr. Grisel had the same risk seeking tendencies as she discussed during her early path into drug addiction. She discusses the risk factors for becoming an addict and the current science behind them - genetics, epigenetics, early exposure, and cultural factors. She discusses the clear link between family history of drug and alcohol abuse and addiction. I experimented with marijuana in my early adulthood (prior to pregnancy), and his father was a lifelong drug addict. Everyone in both our families drank alcohol – some to excess. And like all good addicts, as discussed by Dr. Grisel, my son just couldn't stop once he started. Over the years, he overdosed every time he went into treatment – once visiting a psych unit to detox from meth on the way. After his fifth treatment program at age 23, he had 13 years wonderful years of sobriety. During that time, he got a bachelor's degree in biology and a doctorate in physical therapy (PT). He loved his work as a PT, married a delightful woman and has two lovely children. I truly believe he loved his life, and I know he hated his addiction. He had so much shame and despair around his addiction and past mistakes. Through reading this book, I came to understand how his early drug use changed his brain chemistry so that he would always want, even need, to use something to help him feel normal. He always used nicotine (smokeless tobacco), took Prozac (antidepressant), Valium as needed (benzodiazepine), and sometimes took a medication to help him sleep. Over the past 3 years, he relapsed 3 times. Each relapse was brief – the first ended in his 6th treatment program. He just returned to work after the second one. His last relapse ended in his death. At first, I blamed myself and his father for failing him. Through reading this book, I see the picture is much bigger and more complex. Dr. Grisel explains the science behind every drug my son ingested and why it worked and didn't work, she explains tolerance and why addicts keep taking more and more drug or drugs to reach that high, to feel normal, or just to stave off withdrawal. But more importantly, through her personal story and candor, and her depth of understanding the cultural context, she was able to help me understand that his father and I were part of his story but not the whole story. One tragedy was that my son was born in today's world. She helped me understand that his personality would have fitted him well in the 18th century – on the frontier or as an explorer. It failed him miserably when it came to experimenting with drugs. I miss him terribly and find his loss as tragic and needless as any accidental death. And yet I've learned from this book, that with the trajectory my son was on, without a more current and comprehensive intervention, his death by overdose was almost inevitable. To this point, as Dr. Grisel discussed, there is new science to help addicts achieve a more successful recovery. In retrospect, had I been aware that there is more to recovery than 12 step meetings and abstinence, perhaps we could have changed the outcome. I do thank you, Dr. Grisel, for writing this book. And if any readers of this review want to understand the deep science of addiction in all its complexity - this is the book for you. I am so grateful to have found it. I wish I had could have read it earlier.
